Activity Objectives:
• Identify at least five common patient safety hazards specific to the perioperative period and suggest one prevention strategy for each.
• Apply the concept of root cause analysis during perioperative near-misses and sentinel events and explain the importance of higher-level reporting to prevent further patient harm.
• Practice effective team communication and structured handover techniques in perioperative settings and apply the SBAR framework (Situation, Background, Assessment, and Recommendation).
• Describe at least four current national and international guidelines related to patient safety in anesthesia and the perioperative period.
